Therapy notes belong on the clinician's machine, never on a third party server. Notabium is built around that fact, with templates designed for SOAP and clinical structure.
The privacy posture for clinical notes
Three Notabium template families are locked to local processing in the app code: therapy session, legal deposition, and medical consult. Even if you have Notabium Hosted summaries enabled for other meetings, these three never call any cloud LLM. The enforcement is not a settings toggle. It is enforced in the binary.
This means a therapy session transcribed in Notabium produces a SOAP note generated locally by Qwen3-1.7B or Qwen3-4B, on your laptop's CPU or GPU, with no possibility of the content leaving your machine. Cloudflare never sees it. We never see it. Anthropic never sees it. Apple and Microsoft never see it.
The Therapy Session template (SOAP + AI hybrid)
The template includes the structure clinicians actually use:
- Session metadata: patient initials/ID, date, provider, modality, duration
- AI session summary: one line takeaway plus one paragraph executive summary, generated locally
- Subjective (S): patient self report, mood, symptoms, sleep, appetite, stressors
- Objective (O): observable findings, affect, speech, behavior, orientation
- Assessment (A): diagnostic impression, risk assessment (SI/HI), progress vs goals
- Plan (P): interventions, medication changes, referrals, therapy tasks, follow up dates
- Medications: current meds, doses, changes, rationale
- Diagnoses: DSM/ICD codes
- Safety and risk management: safety plan, emergency contacts, hospitalisation considerations
- Next appointment: date, time, modality
- Audit trail: human review flag, reviewer name and timestamp, AI version
HIPAA aligned by design
Notabium does not process PHI on our servers when you use the therapy template. The audio, transcript, and summary all stay on your machine. There is no Business Associate relationship to negotiate because we never become an associate. We are the software you run locally.

Consent
Recording any clinical session requires patient consent in most jurisdictions. Notabium does not make that decision for you. It records when you click Record. We strongly recommend getting written consent before the first session you intend to record.
Pricing
Solo therapist: Pro at $8/mo yearly. Includes the therapy template, local AI summary, PDF exports for chart attachments, and unlimited transcription. Hosted credits are unused for therapy sessions (locked to local) but still available for non clinical meetings.
Group practice: Team at $20 per seat per month for shared template customization and audit logs.
